Credit card charged twice; once accurately, once inaccurately

I was reviewing credit card purchases from last month and noticed that I was charged twice when I booked a TGV from Lyon to Paris on 13 August. I had received a confirmation email for the ticket I bought, but not for the mysterious extra charge of $64.88. On my bank statement, the charge is listed as "SNCF TGV.COM MITRY MORY" and I notice that I am not the first person to have this exact false charge:
Previous people with the same claim have been given an online reimbursement form, but without a Référence Dossier Voyage, it cannot be submitted.

I want this transaction removed immediately, written confirmation of such, and a credible explanation as to what happened.
