How do I get my reimbursement?

We were travelling from Paris Bercy to Moulin Sur Allier on June 22 when our train was involved in a collision with a car. We were sent back to Paris and had to rebook. We received new tickets for 23/6 on the same 9am train and thought that the ticket officer had also changed our return trip from Moulin to Paris by 24 hours. We discovered that those tickets hadn't been updated and had to pay 98 Euros for that journey. How do I get reimbursed for this amount. The ticket officer at Moulin gave me a card that directs me to but it is not clear how to lodge this claim
